Accommodations

The conference will be held at the Great Wolf Lodge, 2501 Great Wolf Dr., Mason, Ohio. A block of rooms has been reserved at a conference rate of $189 (Family Suite and King Business Suite) plus tax. Please make your reservations by calling 866.954.9653 by July 26, 2010 and identifying yourself as a participant of the Midwest Liver and GI Forum with Reservation #1008MIDW. After July 26, rooms may be reserved on a space and rate available basis only.

The conference registration fees are $100.00 for the day includes CME Credits, Continental breakfast, refreshments, lunch and an invitation to the Group Master Tennis Match on Saturday evening. You may register via mail or fax.  Payment can only be made by check or money order only. (Sorry for the inconvenience)

Refunds

If you register for the conference and find you are not able to attend, you will receive a full refund by calling 513.558.7889 or e-mailing Susan.Waters@uc.edu by August 2, 2010.  Substitutions may be made at any time.

Join us for a private evening at the Lindner Family Tennis Center on Saturday evening, Aug. 21. For more information visit www.cincytennis.com. General admission to the ATP and transportation from the Great Wolf Lodge will be provided. (Limit one ticket per participant)
